When someone has a stroke, his or her life and daily activities change suddenly. In the Champlain Region, just over half of stroke survivors admitted to acute care hospitals are discharged directly back to their home. Research suggests that a significant proportion of these community dwelling stroke survivors struggle to resume meaningful activities, to manage household tasks, to travel within the community or even to perform basic self care. Furthermore, nearly 10% of stroke survivors admitted to acute care hospitals have care needs that cannot be managed in their home and require a transfer to a long-term care home. Responding to the needs of stroke survivors living at home and supporting long-term care homes in providing evidence based stroke care are integral pieces of the Ontario Stroke Strategy.
