HSFO Best Practice Guidelines for Stroke Rehabilitation Management

Definition of Stroke Rehabilitation

The Ontario Provincial Reference Group Cites the World Health Organization (WHO) definition of rehabilitation as a goal-oriented and often time-limited process which enables individuals with impairments, activity limitations and participation restrictions to identify and reach their optimal physical, mental and social function level through a client-focused partnership between the family, providers and the community. Rehabilitation focuses on improving the abilities of the stroke survivor to facilitate independence integration.

Ontario Stroke Rehabilitation Consensus Panel 2007

A panel of physician experts, stroke survivors, community representatives, nursing and allied health professional was convened in January 2006 to develop an integrated model of how services should be provided to benefit stroke survivors in Ontario. This panel developed a framework and key stroke system standards for the purpose of provincial policy development, regional planning, evaluation and performance monitoring of stroke rehab services. A final report outlining the Standards and Recommendations for stroke rehabilitation was submitted to the Ministry of Health & Long-Term Care in the Spring of 2007. You can view the report at www.heartandstroke.ca/profed (see stroke section: professional resources: rehabilitation).
