Stroke Resource Sheet
The Stroke Resource Sheet provides family physicians and other health care professionals with a quick and easy reference to community resources frequently used by stroke survivors. It provides information about key determinants of successful community re-engagement following a stroke and community services available in Ottawa, Eastern Counties and Renfrew County for stroke survivors and their families.

Community Care Access Centres (CCAC)
The first step to access community-based services is through the Champlain Community Care Access Centre (CCAC). The Champlain CCAC coordinates in-home services, such as nursing, physical therapy, occupational therapy and personal support to qualifying clients. The CCAC can also help link stroke survivors to alternate services available in the community such as adult day programs, meal delivery services, assistance with shopping or cleaning or transportation assistance. When people are no longer able to manage at home, the CCAC helps them consider other housing options or coordinate admission to a long-term care home

OTTAWA
The Ottawa Community Support Coalition has a bilingual website to help locate community resources such as transportation, caregiver support, day programs, meals, foot care, etc.: http://www.communitysupportottawa.ca/

Family Physicians and Community Health Centres
It is important that stroke survivors have a family doctor in order to monitor their general health, risk factors and medications The College of Physicians and Surgeons of Ontario (CPSO) maintains a database of registered physicians accepting new patients searchable by community and specialty: http://www.cpso.on.ca/
Patients can go to a walk-in family practice clinic or a Community Health Centre in their area even if they do not have a family doctor there. To find a Community Health Centre in Ottawa, go to the coalition website: http://www.coalitionottawa.ca/
Outside the Ottawa area, the CCAC office may be able to help locate a physician.
