The Champlain Regional Stroke Program operates as part of the Ontario Stroke System and is accountable for providing leadership, development, implementation and coordination of stroke care throughout the region and across all points in the spectrum of care (health promotion, primary and secondary prevention, pre-hospital, acute care, rehabilitation and community reintegration including long-term care). We are governed by the Ontario Stroke Network and the Champlain Regional Stroke Steering Committee. The Ontario Stroke Network provides provincial direction and accountability for the continuous improvement of stroke care across the province of Ontario. The Champlain Regional Stroke Steering Committee sets the regional direction for the program and ensures accountability for the continuous improvement of stroke care in the Champlain Region.
Our program achieves its mission through the efforts of an interdisciplinary team that is represented by:

2008-2011 Strategic Plan
This strategic plan incorporates the new vision and mission statements for the Ontario Stroke System and is built around five new strategic directions. The plan links each strategic direction back to the Champlain Regional Stroke Program's core values. We will build on our earlier achievements, and identify gaps to continuously improve stroke care. We will focus strongly on our strategic directions and our ability to measure outcomes over the next three years.
